Pillars In Action: Heritage
Our SJH Community Services and Adult Day Program Staff were invited by Hamilton County to present around Service Innovation as part of their Transformation-at-a-Glance series. The series is a great education resource for providers, county board staff, families, individuals and other stakeholders, to network, ask questions, and discover great ideas on how to improve both services and lives.
Each presenting provider will talk about how they are making change happen around 5 key areas that matter; Assessment, Planning, Implementation, Evaluation, and Service Innovation. They will be offering up real-world ideas and solutions. St. Joseph Home’s presentation will focus on pathways to opportunities and the strong strides they’ve made in providing individual choices to people who are non-linguistic. They also will detail the assessment and planning aspects of how they have approached the crucial initial stages of service transformation for people living in an ICF (Intermediate Care Facility).
We are so very proud of our staff as their dedication and passion to break barriers through education in the community is the driving force of the St. Joseph Home mission and ministry.
